To access the forms below, you will need the Adobe Acrobat Reader. You can submit on-line forms directly; all other forms can be submitted via email as an attachment.

The following instructions only apply if you are using the full version of Acrobat: To attach a file, click "FILE" on the main menu and then choose "SAVE AS" and save the file onto your computer, then rename the file and email it back to us as an attachment to the following address:  Finance.CustomerService@lacity.org.

Please note that by completing a form and submitting it to the Office of Finance in an electronic format, such as email, you agree that the submitted form has the same legal effect, validity and enforceability of a form submitted to us via U.S. mail or in person.
